On the way to the fourth game, the players pass the bodies of the doctor and the corrupt guards strung up for everyone to see. The Front Man's voice explains that these people cheated and ruined the fairness that the games stand for. He promises that every player here is equal and has the same chance. Later, the Front Man assures someone on the phone that the "minor disturbance" will be cleaned up before the VIPs arrive.

The players are told to pair up for the next game. Mi-nyeo begs everyone to pick her, but the men all want strong partners. Sang-woo quickly chooses Ali, saying they'll be unbeatable together. Gi-hun hesitates, and when he hears someone predict the old man will be the one left out, he goes over and asks the old man to be his partner. Ji-yeong, Player 240, asks Sae-byeok to team up because she seems like someone who would say yes. Deok-su pairs up with one of his own men, Player 278. In the end, Mi-nyeo is the only one left without a partner, and the guards lead the others out while she screams after them.

The game hall looks like an old neighborhood, full of alleys and front doors. Each player gets a bag of ten marbles. Then comes the twist: partners have to play against each other. In 30 minutes, using any marble game they like, one partner has to win all ten of the other's marbles. Whoever ends up with nothing dies. The old man is cheerful, calling Gi-hun his gganbu, an old word for a best friend you share everything with, marbles included.

The pairs fall into very different stories. The old man wanders off looking for the house he grew up in, and he seems confused and childlike. They finally play odd-or-even, guessing whether a hand holds an odd or even number of marbles. When the old man starts winning, a desperate Gi-hun takes advantage of his apparent memory problems, telling him he guessed differently from what he really said. Gi-hun gets the marbles, but he hates himself for it. At the very end, the old man holds out his last marble and reveals he knew what Gi-hun was doing the whole time. Gganbu share everything, he says, and gives it away gladly. He finally remembers his name: Oh Il-nam. Gi-hun walks away, and Il-nam is shot.

Sang-woo and Ali play odd-or-even too, and Ali keeps winning. Sang-woo panics. He convinces Ali that they can both survive by pooling their marbles and teaming up against other pairs later. He asks Ali to hand over his pouch for safekeeping while they scout the other teams. Ali trusts him completely. When Ali later opens the pouch, it's full of pebbles. Sang-woo has already turned in all 20 marbles, and Ali, Player 199, is shot without ever seeing his friend again.

Sae-byeok and Ji-yeong spend most of their time talking instead of playing. Sae-byeok explains that her father drowned trying to cross the border from North Korea and her mother was caught in China and sent back, and that her younger brother is waiting at an orphanage. Ji-yeong admits she just got out of prison for killing her father, an abusive pastor who murdered her mother. In the final minutes they agree to settle it with a single throw. Ji-yeong deliberately throws badly. She says Sae-byeok has people to go home to and she has no one, and asks her to find her mother, take care of her brother and visit Jeju Island. Sae-byeok learns Ji-yeong's name before walking away, and Ji-yeong is killed.

Deok-su beats his henchman, Player 278, by throwing marbles at a hole. When the game is over, Gi-hun and Sae-byeok are devastated, while Sang-woo keeps quiet about what he did.
